    Combined occipital, transtentorial, supracerebellar, transsinus approach for pineal region teratoma

    An 11-year-old boy with a large pineal region mass was admitted to our clinic. The preliminary diagnosis was teratoma. Tumor resection was carried out via combined occipital, transtentorial, supracerebellar, transsinus approach. The tumor tissue was completely resected, and no operative complication other than transient Parinaud syndrome was noted. The histological diagnosis was teratoma, grade 2. After three courses of chemotherapy, the patient underwent external irradiation. He remained asymptomatic with no signs of recurrence 15 months after the surgery. The combination of occipital, transtentorial, supracerebellar, transsinus approach provides excellent views and workspace above and below the tentorial notch. Transverse sinus section is not mandatory for this approach, but sectioning of the unilateral transverse sinus and the tentorium along the rectal sinus allows retraction of the falx and the underlying brain to the opposite side. Thus, a much wider horizontal and vertical projection is obtained. This approach enables more extensive tumor removal for large pineal region tumors.

    Intervertebral discitis and epidural abscess formation secondary to repeated epidural injections: A case report

    Objective: In this article our aim was to emphasize the possibility of development of epidural abscess after repeated injections in a patient into the spinal epidural space. Methods: Lumbar magnetic resonance imaging (MRI) was performed due to worsening of low back pain, inability to walk, fever and sweating. Surgical drainage of the abscess was carried out by bilateral L 5 hemi-partial laminectomy. Antibiotic therapy was recommended for two months and neurological examination was normal after 6 months. Results: Two months after repeated injections, the patient was admitted to our clinic with the complaint of low back pain, inability to walk, fever and fatigue. T 2-weighted magnetic resonance (MR) examination of the lumbosacral spine showed spinal epidural abscess at L5-S1 level and concomitant intervertebral discitis. Staphylococcus aureus grew on cultures. Conclusion: The frequent use of invasive procedures into the spinal cord such as epidural injections increases the incidence of abscesses which can cause serious morbidity if recognition of the infectious agent and treatment with appropriate antimicrobial therapy are delayed. © 2006 VSP.

    Third ventricle colloid cyst accompanying Aicardi syndrome: A case report [Aicardi sendromuna eşlik eden üçüncü ventrikül kolloid kisti: Olgu sunumu]

    Aicardi syndrome is composed of the triad of chorioretinal lacunae, seizures and callosal agenesis. A 3-year-old girl was admitted with the complaints of abnormal eye movements and seizures. In fundoscopic examination, chorioretinal lacunae were detected in both eyes. A magnetic resonance imaging of the brain revealed agenesis of the corpus callosum and a mass obstructing third ventricule. Histopathological examination of the cystic lesion in the patient treated surgically demonstrated colloid cyst. Although good tolerance to intracranial cysts has been reported in Aicardi syndrome, the present patient with cystic lesion obstructing the third ventricle is herein discussed under the light of literature findings. © Gülhane Askeri Ti{dotless}p Akademisi 2010.
