    Evaluation of one year response of treatment naive chronic hepatitis B patients to 245 mg/day tenofovir disoproxil fumarate [Naif kronik hepatit B hastalarinin bir yillik 245 mg/gün tenofovir disoproksil fumarate tedavi sonuçlarinin de?erlendirilmesi]

    Objective: In this study treatment response of 201 chronic hepatitis B (CHB) patients to 245 mg/day tenofovir disoproxil fumarate (TDF) was evaluated upon 48th week of treatment. Material and Method: This retrospective study included 201 treatment naive chronic hepatitis B patients who admitted to infectious diseases clinics of Adiyaman State Hospital, Adiyaman 82nd Year State Hospital and Selçuk University Selçuklu Medical Faculty between 01-Jan-2010 and 31-Dec-2011. Indication for biopsy was defined using American Association for the Study of Liver Disease (AASLD) criteria. Patient charts were retrospectively reviewed. Data were entered to SPSS 16.0 package program. Regarding statistical evaluation chi square test was used for cathegorical variables, and T test, Mann Whitney U test was used for continous variables. For analysis of repeated measurements variance analysis for repeated measures was used and significant groups were tested by signed rank test. p<0.05 was considered statistically significant. Results: One hundred thirty eight (68.7%) patients were males, 63 (31.3%) were females, and mean age was 39.9±12.5. HBeAg was positive in 88 (43.8%) and anti-HBe was positive in 113 (56.2%) patients. Median value for HBV DNA was 52920000 (6032800-34x109) IU/mL, ALT was 98.0 (14.0-665.0) u/L. At 24th week HBV DNA level was below 400 copy/ml in 57% of HBeAg positive and 69.5% of HBeAg negative patients and at 48th week in 81.3% of HBeAg positive and 89% of HBeAg negative patients. At 24th week of treatment ALT level normalized in 64.4% of HBeAg positive and 67.3% of HBeAg negative patients and at 48th week in 83% of HBeAg positive and 79% of HBeAg negative patients. ALT, and HBV DNA values significantly decreased relative to controls. Follow up did not reveal any significant change in creatinine, phosphor, calcium and ALP values. Conclusion: We conclude that TDF treatment was effective and safe during follow up. Because long term treatment is needed for CHB, more long term studies with big sample sizes are needed to determine effectiveness and side effects of tenofovir treatment.

    The importance of soluble urokinase plasminogen activator receptor in patients with acute brucellosis [Solubl ürokinaz plazminojen aktivatör reseptörü’nün akut brusellozlu hastalardaki önemi]

    Objective: Brucellosis is a common zoonotic infectious disease especially in Mediterranean countries. Inflammatory markers are elevated during the course of acute brucellosis. C-reactive protein (CRP) is the most commonly used biochemical marker in clinical practice. Soluble urokinase type plasminogen activator receptor (suPAR) is an interesting biomarker which has drawn attention recently. Purpose of this study is to examine correlation between suPAR and CRP levels as markers of infectious disease in patients diagnosed with acute brucellosis. Material and Method: This study included 125 acute brucellosis patients and 50 healthy controls. Pretreatment blood samples were taken from the patients. suPAR levels were measured using ELISA and CRP levels were measured with nephelometry. Results: There was a positive correlation between suPAR levels and CRP, alanine aminotransferase (ALT) and aspartate aminotransferase (AST) (p=0.045, 0.039, 0.040; respectively). When we compared patient and control groups, CRP and suPAR levels were significantly higher than controls (p=0.001, 0.001; respectively). Growth in blood culture was detected in 14 (11.2%) patients. There was not a significant difference between patients who have or did not have growth in blood cultures (p=0.117). In the ROC curve analysis performed for suPAR, area under the curve (AUC) was 93.6% (p=0.001). Sensitivity and specificity were calculated as 84.8% and 86.0%, respectively, when suPAR’s cut-off value was taken as 3.85 ng/mL according to the ROC curve. Conclusion: Results of this study suggest that suPAR, like CRP, is a promising biomarker in acute brucellosis. © 2015, Nobelmedicus. All Rights Reserved.

    The importance of sonication in the diagnosis of prosthetic joint infections [Prostetik eklem enfeksiyonlarinin tanisinda sonikasyonun önemi]

    Objective: The objective of this study is to investigate the efficacy of sonication method used to determine the cause in the diagnosis of prosthetic joint infections (PJI). Material and Method: This study included 30 patients who were operated due to prosthesis infection and as a control group 10 patients whose prostheses were removed due to mechanical reasons and who had no sign of infection. Cultures were prepared from these tissue samples through gram staining and conventional methods. The prostheses removed from the patients were put into the sonication device in sterile water with ringer lactate. After sonication, Gram staining, cultures and polymerase chain reaction (PCR) were made. Results: During the Gram staining done prior to the sonication, microorganisms were found in six patients (20%); after the sonication, microorganisms were seen in nine patients (30%), but this difference was not statistically significant (p>0.05). While agents were found in the cultures of 11 patients (36.7%) that were prepared using the conventional method, agents were found in 20 patients (66.7%) with the sonication method. The rate of detecting the agent in the culture prepared after sonication was statistical significantly higher than in the culture prepared with conventional methods (p=0.004). The sensitivity of PCR was found 63.3%. Conclusion: The sonication method of PJI is basically a procedure performed to increase the detectability of microorganisms. We found in the present study that the sonication method was obviously more precise than conventional methods in the microbiological diagnosis of PJI. © 2017, Nobelmedicus. All rights reserved.

    Level of knowledge and compliance to hand hygiene among health care workers [Sa?lik çalişanlarinin el hi?jyeni? hakkindaki? bi?lgi? düzeyi? ve el hi?jyeni?ne uyumu]

    Objective: This study aims to evaluate the knowledge and attitud of health care workers (HCW) about hand hygiene. Material and Method: This study was conducted on 479 individuals including 106 doctors, 205 nurses, 111 laboratory workers and student, and 57 cleaning staff working at state hospitals in Adi{dotless}yaman. The study was planned by the Infection Control Committee consisting of 6 members. After listing the study participants, 190 subjects were chosen randomly for observation about compliance to hand hygiene. Ten working days after the questionnaire, 190 randomly selected attendants working at clinics, wards, intensive care unit, or laboratories were observed in total 600 hand hygiene indications. Results: There were 205 nurses, 111 laboratory workers and students, 106 doctors, and 57 cleaning staff in the cohort. In general there was no difference in percentage of correct answers, according to professional status or working units, although new graduates (graduated in the last five years) answered questions more correctly. The most known answer was 'gloves must be changed after each patient, and then hand hygiene must be ensured'. The most wrongly answered question was "gloves protect completely". Hand hygiene compliance at 600 hand hygiene indication was found to be 19%. Compliance rate was highest after exposure to body fluids and was lowest after exposure to the surroundings of the patients. 26% of the personnel who washed hands to achieve hand hygiene and 54% of the personnel who used antiseptic solutions utilized the correct method and duration. Conclusion: In this observational study, we demonstrated low knowledge level and noncompliance among HCW with regard to hand hygiene.

    The life - Saving little tip: Intraosseous gas

    A case is presented of an-aerobic osteomyelitis with intraosseous gas resulting in the extension of gas in soft tissue structures in a patient with diabetes mellitus. The finding of intraosseous gas and its extension into the pubic joint and the left anterior abdominal wall is depicted. Intraosseous gas is a rare but worrying finding for osteomyelitis in the absence of a penetrative wound, recent surgery, biopsy or fracture. Copyright 2013 BMJ Publishing Group. All rights reserved.
