Okul öncesi pediatrik hastalarda perioperatif uygulanan aktif ısıtmanın postoperatif ağrı ve titreme üzerine etkisi

| dc.description.abstract | Perioperatif hipotermi, günlük pratikte en sık karşılaılan perioperatif sorunlar arasındadır. Neonatal, infant ve çocuklar vücut ağırlığına göre daha büyük yüzey alanı, baştan daha fazla ısı kaybı olması, sınırlı cilt altı yağ depoları ve zayıf bir termoregülatör kapasitenin olması gibi yetişkinlerde olmayan faktörlerden dolayı daha çok hipotermi riski altındadırlar. Beklenmeyen perioperatif hipotermi yetişkin hastalarda; artmış cerrahi alan enfeksiyon riski, postoperatif hastanede kalış süresinin uzaması ve hasta konforunu bozan titreme ve postoperatif ağrı ile ilişkilendirilmiş olup tüm bunlar maliyetin artması ile sonuçlanır. Perioperatif süreçte ısınma pediatrik hastanın yara enfeksiyonunu, hastanede kalış süresini, pıhtılaşma bozukluğu riskini, titreme ve diğer zararlı etkilerini azaltarak yararlı olacaktır. Literatürde pediatrik hastalarda perioperatif olarak uygulanan ısıtma yöntemlerinin etkinliği hakkında kısıtlı çalışma bulunmaktadır. Çalışmamızda 2-6 yaş arası elektif opere olan hastalardan kriterleri sağlayıp ailesinin ve/veya kendisinin onamı alınanlar çalışmaya dahil edilmiştir. 2 gruba ayrılan hastalardan A grubuna dahil edilenler preoperatif ve postoperatif süreçte sıcak hava üfleyen önlük ile, intraoperatif süreçte ise karbon fiber battaniye ile ısıtıldılar. B grubundaki hastalar preoperatif ve postoperatif süreçte ısıtılmış yeşil örtüler ile intraoperatif süreçte ise karbon fiber battaniye ile ısıtıldılar. Belirlenen aralıklarla hastaların kor ısısı ölçülmüş olup postoperatif ise belirlenen ölçeklerle ağrı, titreme, ajitasyon durumlarına bakılmıştır. Bu çalışmada pediatrik popülasyonda perioperatif uygulanan aktif ısıtma uygulaması ile (preoperatif ve postoperatif dönemde hava üflemeli önlük ile, intraoperatif dönemde karbon fiber battaniye ile) daha yüksek kor ısı değerleri elde edilerek intraoperatif ve postoperatif daha iyi bir ısı homeostazisi sağlanmıştır. Bununla birlikte sağlanan daha yüksek kor ısı değerlerinin postoperatif ağrı ve titreme skorunu azalttığı gösterilmiş olup titremeye bağlı gelişebilen komplikasyonlar perioperatif tüm dönemlerde aktif ısıtma uygulaması ile azaltılabilecektir. | |
| dc.description.abstract | Perioperative hypothermia is among the most common perioperative problems in daily practice. Neonatal, infant and children are at higher risk of hypothermia due to non-adult factors such as greater surface area than body weight, greater heat loss from the head, limited subcutaneous fat stores and a weak thermoregulatory capacity. Unexpected perioperative hypothermia in adult patients has been associated with increased risk of surgical area infection, longer postoperative hospital stay and tremors and postoperative pain that impair patient comfort and resulting in increased cost. Warming in the perioperative process will be beneficial by reducing the pediatric patient; wound infection, length of hospital stay, risk of clotting disorder, tremors and other harmful effects. There are limited studies in the literature about the effectiveness of heating methods applied perioperatively in pediatric patients. In our study, the patients who had made elective surgery and were 2-6 years old, provided the criteria and whose family and / or their consent was obtained were included in the study. Patients were divided into 2 groups and those who were included in group A were heated with apron blowing hot air in the preoperative and postoperative period, and with a carbon fiber blanket in the intraoperative process. Patients in group B were heated with green blankets in the preoperative and postoperative period and carbon fiber blanket in the intraoperative process. The core temperature of the patients was measured at determined intervals, and postoperatively, pain, shivering and agitation were examined with the scales determined. In this study, higher core temperature values were obtained with the application of active warming applied perioperatively (with an air-blown gown in the preoperative and postoperative period, with a carbon fiber blanket in the intraoperative period) in the pediatric population and a better intraoperative and postoperative temperature homeostasis was achieved. However, higher core temperature values have been shown to reduce the postoperative pain and tremor score, and complications due to tremor can be reduced by active warming in all perioperative periods. | |
